The service is for disabled adults, aged 16-65 years, who have continuing physical impairments.
What is DART?
DART is a team of health care staff who work with disabled adults with physical impairments.
What can DART provide?
Assessments and reviews of your healthcare needs - helping you plan how those needs can be best met.
Specific periods of therapy to help you reach health-related goals.
Information & support for you and your carers.
Long term support for you and your carers, as agreed together.
They aim to work closely with other workers and organisations e.g. Social Services, GP's, Voluntary etc.
Who is in the team?
Team manager, Health Care Facilitators, Occupational Therapists, Physiotherapists, Clinical Psychologist, Rehabilitation Assistants, Secretaries and a Speech and Language Therapist.
Where does DART see people?
They try to visit people wherever it is best for everyone concerned. This is usually at your home but could be somewhere else, e.g. at work or at college.
Anyone can make a referral to the service, including you or you carer.

DART now has waiting lists, but they will let you know when they hope to respond to your request for help.
